What are your operating hours?
Employee Services operates Monday through Friday between 8:00 a.m. - 5:00 p.m.
Where is Employee Services located?
We are located at 320 N. Elizabeth on the first floor. We're at the corner of Elizabeth and Carroll Ave., just one block north of Lake St. For more information, call (773) 553-1142.
What is your fax number?

How do I get to your office?

Where can I download the Enrollment Forms?
They can be accessed through sections home pages as follows:
Where can I go to get a Drug Test and TB Test?
You can visit any Mercy Works location for your Drug Test. For your TB test you may visit, any Mercy Works location, your general physician or contact a physician's office from the City of Chicago Health Clinics.
How can I schedule an appointment for my enrollment session?
Please contact (773) 553-1142 for scheduling an appointment with an enrollment coordinator.
Teachers Only
What documents should I bring to the Enrollment Unit?
Please bring the following items to be processed:
- Proper I-9 Identification. Examples include: Passport, Driver's License or State ID and SSN Card or Birth Certificate. See the New Hire Packet for a list of acceptable forms of ID.
- Original Transcripts for all degrees conferred or graduate level courses to be credited for Lane Change.
- Teaching Certificates
- DS2 Recommendation Staffing Confirmation Email.
- Completed Drug test and TB test.
Where do I go to renew/obtain my certificate?
To renew/obtain your certificate contact Illinois State Board of Education, Educator Certification, 100 N. First Street S-306, Springfield, Illinois 62777. If you have questions or concerns regarding certification-related matters, please contact the Springfield office from 8:00 a.m. to 5:00 p.m. Monday - Friday at (217) 557- 6763.
What is the salary schedule for teachers?

What documents should I bring to Employee Services?
Please bring the following items for your enrollment session:
- Proper I-9 Identification. Examples include: Passport, Driver's License or State ID and SSN Card or Birth Certificate. See the New Hire Packet for a list of acceptable forms of ID.
- High School Diploma or GED (all Educational Support Personnel positions, except lunchroom attendants and porters).
- DS2 Recommendation Staffing Confirmation Email
- Completed Drug test and TB test
- If you are being hired in one of the ESP Paraprofessional job titles listed below:
- The ESP Paraprofessional candidate needs two (2) sets of transcripts (1 for Illinois State Board of Education and 1 for Chicago Public Schools).

- Teacher Assistant I & II
- Teacher Assistant Montessori I & II
- Teacher Assistant Bilingual I & II
- Teacher Assistant Bilingual-Spanish I & II
- School Library Assistant I & II

- Before visiting 320 N. Elizabeth, all ESP Paraprofessionals must obtain a No Child Left Behind Statement of Approval Form from the Illinois State Board of Education (ISBE) Educator Certification, 100 N. First Street S-306, Springfield, Illinois 62777. ISBE hours of operation are Monday - Friday from 8:00 a.m. - 5:00 p.m. You can reach ISBE via web at www.isbe.net or phone at (217) 557-6763.
