General Questions

What are your operating hours?

The Employee Service Center-Enrollment Unit operates Monday through Friday between 8:30 a.m. - 5:30 p.m.

Where is the Employee Service Center-Enrollment Unit located?

We are located at 320 N. Elizabeth on the first floor. We're at the corner of Elizabeth and Carroll Ave., just one block north of Lake St.  For more information, call (773) 553-1142. 

What is your fax number?

How do I get to your office?

Please view our Directions page for public transportation and driving directions to the Enrollment Unit.

Where can I download the Enrollment Forms?

They can be accessed through our New Hire Forms page.

Where can I go to get a physical and TB Test?

You can visit your general physician or contact a physician's office from the City of Chicago Health Clinics.

How can I schedule an appointment for my enrollment session?

Teachers Only

What documents should I bring to the Enrollment Unit?

Please bring the following items for your enrollment session:
  • 2 Forms of ID (driver's license or state id and social security card or birth certificate). Note: A passport counts as 2 forms of id
  • Official Transcripts (no copies will be accepted)
  • Copy of Teacher Certificate (candidates can visit www.isbe.net/otis to obtain their certificate information)
  • Completed CPS Physical Examination Form
  • Voided Check (optional - for direct deposit only)
Where do I go to renew/obtain my certificate?

Please visit the James R. Thompson Center, 100 W. Randolph, Suite 14-200, Chicago, Illinois. Their hours of operation are Monday - Friday between 10:00 a.m. - 4:30 p.m. For further information, please visit or call (312) 814-8113.

What is the salary schedule for teachers?

Educational Support Personnel Only 


What documents should I bring to the Enrollment Unit?

Please bring the following items for your enrollment session:
  • Proper I-9 Identification. Examples include: Passport, Driver's License or State ID and SSN Card or Birth Certificate. See the New Hire Packet for a list of acceptable forms of ID.
  • High School Diploma or GED (all educational support personnel positions, except lunchroom attendant and porters)
  • The following ESP Paraprofessional Job Titles must posses a minimum of 60 semester hours or at least 90 quarter hours, or posses an Associate's Degree or successful completion of the WorkKeys® test or the ParaPro® assessment test:
    • School Assistant
    • Teacher Assistant
    • Special Education Classroom Assistant
    • School Assistant Bilingual-Spanish
    • School Assistant Bilingual
    • School Library Assistant
    • Teacher Assistant - Montessori
    • Instructor Assistant
  • Official Transcripts (two sets of transcripts - one set for CPS and the other set for ISBE, along with a High School Diploma to apply for the NCLB Statement of Approval. This is for ESP Paraprofessional Candidates only)
  • Completed CPS Employee Physical Examination Form (the form can be submitted after the fingerprints have cleared)
  • Voided Check (optional - for direct deposit only)
