Attention Items
Without the following documents, you will be unable to complete the New Hire / Rehire process.
- Two Forms of Identification:
- Driver's License or State ID. A driver's license/state ID are considered one and the same. If a driver's license or state ID is used, then another form of ID is required, such as a birth certificate or social security card.
- A Passport is considered as two forms of ID.
- A Completed Drug Test and TB Test (if a TB test is not possible, a Chest X-Ray report must accompany the physical). Please be sure the doctor's ID number is on the CPS Physical Form or the form will not be accepted. Drug testing can only be done at Mercy Works locations that are listed on the form. The fingerprint based background check is done at 320 N Elizabeth, 1st Floor.
- Completed New Hire Package.
- Original Transcripts for each degree you hold.
- High School Diploma (Educational Support Personnel).
- NCLB Approval or Paraprofessional Letter of Approval (Educational Support Personnel).
- Teaching Certificate.
- DS2/Referral Request Form (including rate of pay and bucket number on Miscellaneous Referral Request Form).
